Medical Paperwork and Screening
Every student-athlete must complete and submit appropriate documentation to the Sports Medicine Staff before they may begin any official activity with an intercollegiate athletic team. Paperwork is to be completed online using the Athletic Trainer System. Once online paperwork is completed, you must schedule an appointment with your team's Athletic Trainer to complete an in-person screening. Without the in-person screening, you are not cleared to participate.

Incoming student-athletes who are new to St. Joseph's University, Long Island (freshman or transfer) must also provide a physical from their primary care physician and Sickle Cell Trait test results.
